FIELD: medicine.
SUBSTANCE: device for fixing at least one intervertebral implant in at least one vertebra is intended for insertion from the peripheral region of the spine through a channel extending through at least a portion of the implant. The device has a body comprising at least one rigid plate elongated along the longitudinal axis (L) extending between the front end and the rear end and configured so that its front end enters at least one vertebra and its trailing end remains in the implant channel, which ensures that the implant is kept close to the vertebra. The plate comprises at least one slit hole oriented substantially parallel to its longitudinal axis (L) and separating at least the plate rear portion into two branches. The slit hole is formed relative to the plate rear portion, so that the two branches can be brought together when pressure is applied to them. The intervertebral implant comprises a body where at least one portion, called the rear portion, comprises at least one channel configured to receive at least one of the abovementioned fixing means to allow passage of the rigid fixing device without deformation in spite of it curvature. The above device has at least one of the branches with at least one extraction stop oriented not parallel to the longitudinal axis (L) of the plate and configured to interact with the complementary implant surface and to retain the device in the implant. The channel extends through the implant from the peripheral region to the top or bottom surface along the rectilinear and inclined paths adapted to the curvature of the fixing device inserted substantially in the implant plane to orient the fixing device towards the end plate of one of the vertebrae between which the implant is to be installed. The channel comprises at least one surface complementary to at least one stopper for extraction of the above-mentioned fastening device. A tool for the said intervertebral implant implantation between the adjacent vertebrae and for implantation of at least one of the said fixing device, referred to as an anchor, to at least one of the said vertebrae, comprises at least one holder, at least one actuating member and at least one guide. The holder comprises a body having a width less than the width of the anchor and comprises at least one guiding surface having at least one radius of curvature substantially identical to at least one radius of curvature of the anchor plate for positioning and guiding this anchor during implantation. One actuating member comprises a head configured to receive the holder between two arms longer than the length of the holder body and spaced apart from each other by a distance greater than the width of the holder body or equal thereto, by moving the actuating element along the holder, the anchor held by the holder. The guide has an elongated shape along the longitudinal axis extending between the first end, called the gripping end, to grip the implant, and the second end, called the pushing end. The gripping end has a head provided with at least one gripping structure at its end for interaction with at least one retaining structure of the implant. The head intersects the longitudinal channel leading to the implant and having a shape and dimensions suitable for receiving at least a part of the body of the holder and the actuating member arms. The head channel includes at least one surface to guide the fastening device that is complementary to the guide surface of the holder, to guide the fastener between the two guiding surfaces of the guide and the holder, while the actuating member is moved along the holder to the guide head.
EFFECT: simplified implantation, in particular along the axis, essentially perpendicular to the spine axis, and more secure fixation with reduced risk of damage to the vertebrae, particularly during implantation through rear and/or transforaminal access.
