SUBSTANCE: invention relates to medicine, namely to ophthalmology. Surgical management of open-angle glaucoma comprises dissecting a conjunctival flap, a surface scleral flap (SSL), removing sclera middle layers before exposing inner layers of the trabecula corneoscleral part and Descemet's shell limbic margin, implantating Glautex drainage, and repositioning of the SSL. For this purpose, a rectangular SSL is cut via cornea surface layers with a 3.5 mm base near the limb and a height of 4 mm. Sclera middle layers are removed along the SSL bed edges, corresponding to its height, keeping the sclera middle layers in the middle of the bed in the form of a 1 mm strip. Glautex drainage is implanted by 2.5×5.2 mm cuts under the SSL with a smaller base toward the limb, in front of the Schlemm channel. On the sides of the SSL, scleral 0.5 mm strips are cut along the entire length and after repositioning the SSL a continuous seam is applied to the conjunctiva above it and fixed to the SSL.

EFFECT: method provides higher efficiency of surgical treatment of secondary and primary open-angle glaucoma by decreasing postoperative complications such as eruption and disruption of the conjunctival suture in the early postoperative period, and achieving a prolonged antihypertensive effect with the formation of a spilled filter cushion.
