FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to surgery, and can be used as a method for assessing the severity of peritonitis. Essence of the method: in patients, the level of the binding reserve of albumin and the level of malonic dialdehyde are determined in venous blood. Microcirculation index is estimated by laser Doppler flowmetry using a laser microcirculation analyzer. Indices are determined before the operation and daily, starting from the first day after surgical treatment of the patient, in dynamics. Based on the data obtained, the peritonitis severity index (ITP) is calculated from the formula where RSA1 – level of the binding reserve of albumin at the current moment, RSAn – level of the binding reserve of albumin is normal, PM1 – indicator of microcirculation at the current moment, PMn indicator of microcirculation is normal, MDA1 – level of malonic dialdehyde at the current moment, MDAn – level of malonic dialdehyde is normal. At the same time ITP value of less than 3.4 indicates an easy degree of severity of peritonitis and ascertains the adequacy of the therapy, ITP value from 3.5 to 4.9 indicates a severe degree of peritonitis, assumes a high degree of probability of progression of peritonitis and causes the correction of therapy schemes and the appointment of efferent methods of detoxification or repeated surgical interventions, value of ITP more than 5.0 indicates a critical severity and states the progression of peritonitis with presence of purulent exudate, which determines the need for repeated surgical interventions.

EFFECT: invention allows to improve accuracy of the evaluation of the severity of peritonitis on the basis of determining the level of toxic products in the local and body blood streams, which allows to take timely measures to correct therapy.
