FIELD: medicine; physics.
SUBSTANCE: invention relates to medicine, radiology. Determination of the level of PSA in blood, chemotherapy and radiotherapy with a split course in the mode of dose fractionation is carried out. Determine quantitative morphological signs indicating the differentiation of tumor cells on the Gleason scale. Detect PSA values within 10 ng/ml <PSA≤20 ng/ml and the level of differentiation of the tumor on the Glisson scale ≤7, the area of the first stage of the split irradiation course includes the external iliac and obturator lymph nodes on both sides. At the same PSA value and the level of differentiation of the tumor on the Glisson scale >7, the common, external, internal iliac lymph nodes and obturator lymph nodes are included in the irradiation zone. When revealing the value of PSA>20 ng/ml and the level of differentiation of the tumor on the Glisson scale ≤7 – external iliac, common iliac, internal iliac, obturator and pararectal lymph nodes are included in the area of the first stage of irradiation. With PSA>20 ng/ml and Glisson's index >7 – common, external, internal iliac, obturator, pararectal and para-aortic lymph nodes are also included in the amount of exposure. Radiation therapy is carried out up to SOD 40 Gy at the first stage of the split course, and irradiation for the entire course is 68–74 Gy.
EFFECT: method allows to achieve high rates of life expectancy, an extremely low degree of damage to critical organs located in the immediate vicinity of the prostate gland, which will significantly improve the patient's quality of life.
