FIELD: medicine.
SUBSTANCE: invention refers to medicine, particularly to oncology and pathological anatomy, namely to differential diagnostics of large neoplasm. That is ensured by morphological, immunohistochemical and molecular-genetic studies of the removed new growths, and simultaneous detection in the remote formation filled with paraffin blocks, as differential diagnostic criteria of types of toothed new growths presence or absence of mutations 2, 3, 4 exon KRAS gene and mutation gene BRAFp.V600E by polymerase chain reaction in real time mode and features expression mucins-2, 5AC and -6 by immunohistochemistry depending on presence or absence of mutations of KRAS and BRAF genes. Sinatomal adenoma is diagnosed by absence of mutation of KRAS gene and expressed expression of mucine-2, expression of mucin-6 in dilated bases of crypts, as well as high expression of mucine-2 and expressed positive expression of mucin-5AC in the cytoplasm along the length of the crypt in the presence of mutation of the BRAF gene, or absence or focal moderate expression of mucin-5AC along the length of the crypt in the absence of mutation of the BRAF gene. Diagnosis of traditional jagged adenoma is stated by absence of mutation of gene BRAF, absence of expression mucine-6, as well as expressed expression of mucine-2 along length of crypt or moderate expression of mucine-2 in crypt bases and absence of expression of mucin 5AC in absence of mutation of KRAS gene or focal expression of mucin 5AC with arrangement in upper part of crypt and superficial epithelium along with pronounced mucine-2 expression along crypt length in presence of mutation of KRAS gene. Diagnosis of hyperplastic polyp is stated by presence of expressed cytoplasmic expression of mucin-2 along length of crypt, negative expression of mucin-6 in bases of crypts and focal positive expression of mucin-5AC in dilated bases of crypts irrespective of presence of mutations of genes KRAS and BRAF.
EFFECT: invention provides a morphological diagnosis, allowing to differentiate all three types of large neoplasm of large intestine, as well as the invention enables not only to diagnose when a patient enters a health care facility, but also over time if it is necessary to specify the diagnosis or to verify the diagnosis.
