FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to ophthalmology, and can be used for reducing a risk of intraoperative hemorrhagic complications in children with congenital cataract combined with a syndrome of primary persistent hyperplastic vitreous body. At stage of preoperative diagnostics fluorescent angiography (FAG) of hyaloid artery striae is performed. If observing a site with active blood flow, an ultrasonic examination of a hyaloid artery strand in color Doppler mapping is performed. Length of the avascular tendon of a hyaloid artery is determined starting from a point of attachment of a hyaloid artery to a posterior capsule of a lens. During the surgical intervention, the posterior capsulorhexis stage is performed using a femtosecond laser. Laser-cut disk in the posterior capsule of the lens has a rounded shape, is localized in the area of projection of attachment of the hyaloid artery to the posterior capsule of the lens. Diameter of laser-cut disk in posterior capsule of lens makes it possible to cover maximum area of area of projection of attachment of hyaloid artery strand to posterior capsule – from 3.0 to 3.5 mm. Hyaloid artery diathermocoagulation is performed within its avascular zone at 2/3 length of avascular segment, starting from attachment point of hyaloid artery to posterior capsule of crystalline lens. Then the artery is crossed in a diathermocoagulation point.
EFFECT: method provides eliminating the risk of intraoperative hemorrhagic complications in the surgical management of congenital cataract combined with the syndrome of the primary persistent hyperplastic vitreous body in children due to laser cutting of a disc in posterior capsule of crystalline lens with diameter from 3 to 3½ mm and diathermocoagulation of hyaloid artery strand within its avascular zone at 2/3 of length of avascular segment.
