FIELD: medicine; ophthalmology.
SUBSTANCE: posterior capsulotomy is performed using a femtosecond laser. After performing paracentesis and the main tunnel incision, an anterior femtolaser capsulotomy with a diameter of 4.0 mm is performed. The contents of the lens bag are removed, the surface of the posterior lens capsule is polished, fibrosis of the posterior capsule is visualized, after which the instruments are removed from the eye. In this case, the volume of the anterior chamber decreases due to the outflow of fluid from it, and the posterior lens capsule occupies an arbitrary position. After that, the volume of the anterior chamber is replenished with heavy viscoelastic so that the posterior capsule occupies a position not lower than 1.0 mm from the level of the iris, and not higher than the level of the iris, the paracentesis and the main incision are hydrated, and when the position of the posterior lens capsule is reached, which is controlled by intraoperative optical coherence tomography on the built-in femtolaser optical coherence tomograph, after which posterior femtolaser capsulotomy is performed.
EFFECT: method makes it possible to safely perform posterior femtolaser capsulotomy during phacoaspiration of congenital cataract in children with clouding of the posterior lens capsule, restore transparency of optical media, eliminate the risk of intra- and postoperative complications, increase visual acuity in the postoperative period in the absence of gross changes in the posterior segment of the eye.
