FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to neurosurgery, and can be used for surgical management of the patients with bone defects of the cranial vault. A super-elastic four-layer mesh knitted implant repeating the configuration of the bone defect, made of nickel-titanium filament with thickness of 30.0–40.0 mcm with a microporous oxide layer of up to 7.0 mcm by triple braiding with mesh size of 150–200×250–300 mcm, are placed on the bone edges of the defect under the periosteum, between the dura mater and the outer tissues, without tension, with an outer covering of 3.0–5.0 mm. Fixing along the perimeter with mini brackets of titanium nickelide with shape memory effect in the form of open rings with length of 6.0–7.0 mm and leg length of 3.5–4.0 mm, made of wire with diameter of 0.8 mm. Two outer layers of mesh implant material are pre-saturated with osteogenic tissue grown in thickness of comb of iliac bone, having a structure between hyaline cartilage and coarse fibrous bone tissue containing low-grade mesenchymal cell elements. To restore shape of skull as stiffness rib there used is one, two or more plates from porous nickelide of titanium, repeating shape of skull, corresponding to length of defect, with width of 5.0–15.0 mm, thickness of 0.1–0.2 mm, laid in subperiosteal, external mesh structures, resting on bone edges of defect by 3.0–5.0 mm, pre-saturated with osteogenic tissue. It is fixed with mini staples from titanium nickelide to recipient skull bones.
EFFECT: method enables effective elimination of cranial arch defects by using a super-elastic mesh nickelide-titanium implant, formation of an internal connective tissue reagent integrated with the implant material, use of fixing structures in the form of mini-clamps from titanium nickelide, having shape memory effect.
