FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to surgery and oncology, and can be used for preoperative determination of volume of surgical treatment of high-grade thyroid cancer. A fine-needle aspiration biopsy (FAB) is performed under ultrasound control. Additionally calcitonin level in blood is determined. If the FAB results are used to determine a follicular tumor, cell atypia, Hurthle cells, additionally ultrasonography is used to determine signs of oncological affection, such as contour unevenness, intensified hypervascularization of thyroid gland according to Doppler sonography. If the above signs are detected, the first stage involves hemithyroidectomy, the biopsy material of the distant portion is examined. If the pathohistological examination is used to determine medullary cancer and the calcitonin level is increased, the second stage involves a second operation involving thyroidectomy and bilateral central lymph node dissection. If pathological histopathological examination of medullary cancer cells biopsy material shows no calcitonin level, surgical management is limited by hemithyroidectomy followed by conservative treatment.

EFFECT: method provides a preoperative determination of the volume of surgical treatment of high-differentiated tumors localized in one thyroid lobe, with determination of further tactics after the obtained histological response: papillary, follicular or medullary carcinoma by analyzing certain factors on the basis of which one or another therapeutic approach is selected, followed by patient observation.
