SUBSTANCE: invention relates to medicine, namely to abdominal surgery. During the orthotopic liver transplantation, the patient is examined a volumetric blood flow for hepatic arterial (VBF HA) and portal vein (VBF PV) volumetric blood flow directly after blood transfusion in the transplanted liver. If the VBF HA value is less than 100 ml/min, the paravasal tissues are infiltrated with papaverine, the hepatic artery is dilated with a Fogarty probe, the gastroduodenal artery is ligatured, and the transplant perfusion is evaluated with allowance for the perfusion index (PI) calculated by the original formula. If the PI value is less than 0.13, the splenic artery is clamped for no more than 10 minutes, while measuring VBF HA and VBF PV. After that, PI is calculated again and if its value is less than 0.13, aorto-hepatic arterial anastomosis is formed.

EFFECT: method enables avoiding hepatic arterial thrombosis and ischemic complications in post-transplant recipients with pre-transplantation anamnesis, as well as maintaining VBF on PV within normal limits, while increasing the length of liver transplant operation.
