FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to traumatology, orthopedics and oncology, and can be used in surgical management of the patients with new growths, posttraumatic defects, osteomyelitis, false joints of the lateral malleolus. Bone autograft is taken with a fibular artery and a vein from a contralateral limb. External collateral ligament of the knee joint is anchored to the tibia. Then microvascular anastomosis is applied between the fibular vessels of the bone autograft and the fibular vessels of the involved extremity end to end. Head of the fibular bone of the autograft is covered with a segment of its surae fascia by turning it distally. Autograft is fixed to the preserved part of the fibrous bone with a reconstructive plate with angular stability. Then, the constricting screw is simulated by the tibiofibular syndesmosis. That is followed by the reconstruction of the external collateral ligament of the ankle joint by the tendon of the short fibular muscle, conducting the tendon graft on the proximal base anteriorly through the canal formed in the autograft head and returning it backwards through the canal formed in the ankle bone. End of tendon is anchored to it by end-to-side type.

EFFECT: method enables preserving the recovered talus-fibular joint biomechanics and reducing the risk of arthrodesis, providing ankle joint stability by using plastic material from contralateral limb, as well as by using short fibular muscle tendon having sufficient strength and length for reconstruction of external lateral ligament of ankle joint, facilitation of surgical technique ensured by compliance of crossed vessel diameters and simultaneous implementation of the claimed method.
