FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to maxillofacial surgery, and can be used in the surgical treatment of angular fractures of the mandible with displacement of bone fragments in the presence of partial or complete adentia. Computed 3D tomography, assessment of the condition of the lower jaw, determination of the nature of the fracture are performed. Under conduction anesthesia on the medial fragment, departing from the fracture gap 1-1.5 cm, with the help of a mucotome, the mucous membrane is excised at the insertion sites of dental implants on the alveolar process of the lower jaw, then the beds are formed with bone cutters and the required number of implants is inserted, but not less than two implants. On the abutments, using dental cement, crowns are fixed, on the vestibular surfaces of which the retainers for the wire with threaded holes for the screw are soldered. The fragments are repositioned in the anatomically correct position. Next, a needle is inserted into the distal fragment into the anterior edge of the branch of the lower jaw, after perforation of the bone, the needle is advanced to a depth of 1.5-2 cm for its reliable fixation in the bone, the free end of the needle is bent parallel to the alveolar ridge and inserted into the retainers for the needle with threaded holes, tighten the screws in the threaded holes of the retainers. After 3-5 weeks, a control computed 3D tomography is performed, after the consolidation of the fracture of the lower jaw, the fixing screws are unscrewed, the wire and crowns are removed from the heads of the implants. In the absence of rarefaction of the bone tissue in the area of the root part, the implants are used for further prosthetics.
EFFECT: method allows providing proper compression between the fragments and their stable fixation in the anatomically correct position, which excludes displacement of bone fragments at an angle, lateral displacement, displacement along the length, displacement along the periphery due to rotation of the fragments around the axis (rotational), makes it possible to atraumatic and technically easy removal of the entire structure after consolidation of the fracture, a one-step solution to the problem of edentulous lower jaw.
