FIELD: experimental medicine.
SUBSTANCE: invention relates to experimental medicine, namely to military field surgery, injury surgery, abdominal surgery, disaster medicine. The pig under endotracheal anesthesia is placed on its back with divorced and fixed limbs. Introduce sheaths 4Fr in the common carotid artery, 5Fr in the femoral artery and 6Fr in the femoral vein. A pulse oximeter is fixed in the tail area. An epicystostomy is applied. With the help of an electromechanical pump, controlled blood loss is carried out in the amount of 40% of the circulating blood volume. In the area of the anterior abdominal wall, the pneumatic chamber is positioned in the projection of the infrarenal aorta, preliminarily placing strain gauges under the chamber. The pneumatic chamber is pumped until objective indicators of effective compression of the large vessels of the abdominal region appear. The criterion for sufficient compression is considered an increase in systemic blood pressure, heart rate and respiratory excursions, the disappearance of the pulse wave during palpation and the Doppler signal during angioscanning of the vessels of the lower extremity, a decrease in blood oxygenation in the lower half of the trunk, and the disappearance of data from the sensor for invasive pressure measurement placed on the femoral arteries, lack of blood flow in the descending part of the aorta, inferior vena cava according to angiography. Aorto-cavagraphy of the infrarenal zone is performed.
EFFECT: method allows creating a successful experimental model for evaluating the effectiveness of stopping intra-abdominal and external bleeding in the ilio-groin area using external compression devices of local action, is minimally invasive, and allows training medical personnel to practice effective use of the compression device.
