SUBSTANCE: invention relates to medicine, namely to internal medicine, cardiology and nephrology and is intended for predictive and early diagnosis of acute kidney injury (AKI) in patients with acute coronary syndrome (ACS). To implement the invention, Mehran score is used, which includes 8 parameters, for each of which a certain number of points is assigned: systolic blood pressure decrease of less than 80 mm Hg – 5 points, use of intra-aortic balloon counterpulsation - 5 points; acute heart failure of the III and IV stages of Killip or pulmonary edema upon admission - 5 points; age > 75 years - 4 points; a decrease in hematocrit (less than 0.39 for men and 0.36 for women) - 3 points; diabetes mellitus - 3 points the volume of the contrast agent is 1 point for every 100 ml; reduction of GFR less than 20 ml/min - 6 points, 21-40 ml /min - 4 points, 41-60 ml/min - 2 points, and additionally determine the level of soluble tumorigenesis suppressor-2 (sST2) and at a level of sST2 more than 27.2 ng/ml additionally assign 2 points. With the total amount of assigned points above 8 points, a high probability of developing AKI is predicted.
EFFECT: method makes it possible to increase effectiveness of predicting AKI in patients with ACS taking into account the main pathogenetic mechanism of AKI development in patients with ACS, increase prognostic capabilities especially of patients with ACS to provide them with timely medical assistance.
