FIELD: surgical instruments.
SUBSTANCE: invention relates to surgical instruments and can be used for sternotomy, for various types of chest deformity, as well as for making access in case of cardiac diseases. A set of surgical instruments for sternotomy consists of two instruments, one of which is a guide plate with a bent upwards, rounded working end, and a longitudinal groove along the axis. The guide plate is equipped with a movable handle mounted on a shank in the form of a washer, raised above the surface of the guide plate, and separated from it by a vertical shoulder covering the shank to the width of the said plate. Along the outer, peripheral, contour of the upper surface of the shank, three equal-sized rectangular grooves are radially located, one of which is along the axis, and the other two are on both sides of it, at an angle of 45 degrees to the longitudinal axis. On the lower surface of the base of the movable part of the handle, also in the form of a washer, at the border with the handle, a longitudinal protrusion is made, in shape and size, corresponding to the grooves on the plate shank. The shank and the base are interconnected by a fixing screw installed in the hole in the center of the washers of the plate shank and the base of the handle. At the end of the screw rod, a restrictive washer is installed and fixed with a screw of a smaller diameter, placed in a cylindrical recess on the lower surface of the plate shank. The second tool is chopping, with a working part in the form of a vertically located blade with a cutting edge, the back side of which passes into the neck of the handle, forming an obtuse angle with the horizontal surfaces of the blade and handle, at the free end of which there is a striker. At the base of the neck of the handle, parallel to the cutting edge, a through threaded hole is made, in which, from the side of the lower surface of the chopping tool, a T-shaped protrusion is installed and fixed for installing and fastening the chopping tool into the longitudinal groove of the guide plate. The T-shaped protrusion has a thread at the end of the leg, while the height of the protruding part of the leg corresponds to the thickness of the guide plate, the diameter of the protruding part of the leg and the width of the protrusion head correspond to the width of the groove in the guide plate, the length of the protrusion head exceeds the width of the groove. At the end of the leg there is a slot for a screwdriver, which is located along the protrusion head. On the upper surface of the blade, above the cutting edge, and perpendicular to it, a convex restrictive plate is installed and fixed with a screw, the edges of which protrude beyond the side contours of the blade.
EFFECT: development of instrument design that reduces trauma during surgical treatment of keeled, funnel-shaped, deformity of the sternum and in the implementation of transsternal access during cardiac operations.
