FIELD: health care.
SUBSTANCE: invention relates to health care, namely ophthalmology, and can be used for laser surgical treatment of patients with the initial and advanced stages of primary open-angle glaucoma. A diagnostic examination of the patient is performed for this purpose, including gonioscopy to identify the degree of exogenous pigmentation and optical coherence tomography of the anterior segment of the eye along a 360-degree circumference to identify areas of sclerosis of the Schlemm canal and localization of collector veins relative to the Schlemm canal. Next, selective laser trabeculoplasty is performed along a circumference of up to 180° in areas with the most pronounced exogenous pigmentation. Then, YAG laser trabeculopuncture is performed no closer than 1.5 mm from the projection of the collector veins on the trabecula in the areas of absence of sclerosis of the Schlemm canal until the first appearance of a hemorrhagic point in the trabecula. Parameters of trabeculopuncture: wavelength 1064 nm, spot diameter 50 microns, initial pulse energy 1.5 MJ. YAG laser trabeculopuncture is performed at 10-15 points of the trabecula.
EFFECT: present invention enables to steadily decrease the intraocular pressure an hour after surgery by improving the outflow of intraocular fluid by affecting both links of the pathogenesis of primary open-angle glaucoma; reducing the pigmentation of the trabecula due to photothermolysis of pigment granules and the formation of trabeculostomes in the trabecular mesh, taking into account the location of collector veins in relation to the Schlemm canal, and also provides prevention of hyphema in the postoperative period.
