METHOD FOR SURGICAL TREATMENT OF AN OSTEOCHONDRAL DEFECT OF THE HUMERAL HEAD IN CHRONIC POSTERIOR DISLOCATION OF THE SHOULDER Russian patent published in 2023 - IPC A61B17/56 

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and can be used for surgical treatment of an osteochondral defect of the humeral head. Prior to isolating the humeral head, the tendon of the subscapularis is dissected along the fibers until the articular surface of the scapula is visualized. After removal of the scar tissue and vivification to subchondral bone tissue in the area of the defect, its dimensions are determined. The iliac crest is accessed and the autograft is taken according to the determined defect size, after which one lateral cortical layer of the autograft is removed and the autograft with the resected surface is placed against the medial wall of the defect so that the upper cortical layer of the autograft continues the curvature of the articular surface of the humeral head. The autograft is fixed with screws to the head of the humerus through the lateral cortical layer, perpendicular to its longitudinal axis. The upper cortical layer of the autograft is modeled according to the anatomical curvature of the articular humeral head surface. Mylar threads are fixed by the caps of the installed screws, and then the dissected part of the tendon of the subscapularis is sewn with these threads.

EFFECT: method provides increased efficiency of treatment of chronic humeral head dislocation by restoring the congruence of the articular humeral head surface with the articular surface of the scapula and restoring the stabilizing function of the subscapularis muscle.
