METHOD FOR POSTERIOR LAYERED KERATOPLASTY IN CASE OF ENDOTHELIAL DYSFUNCTION OF THE CORNEA WITH VIOLATION OF THE INTEGRITY OF THE IRIDOLENTICULAR DIAPHRAGM Russian patent published in 2023 - IPC A61F9/07 A61F9/08 

FIELD: medicine; ophthalmology.

SUBSTANCE: using a femtosecond laser, a bandage stromal flap (BSL) with a diameter of 9 mm and a thickness of 200 μm is formed from a donor corneoscleral disc. In this case, the resection of the BSL is carried out from the side of the Bowman's membrane. The obtained BSL is implanted into the anterior chamber of the eye so that the stromal surface of the BSL faces the posterior stromal surface of the recipient's cornea. Then, the donor posterior layered graft is implanted in the space between the posterior surface of the recipient's cornea and the BSL under the action of a constant supply of balanced saline, which provides the volume of the anterior chamber and pushes the BSL down. Using intraoperative optical coherence tomography (OCT), the position of the posterior layer-by-layer graft relative to the stroma of the recipient's cornea and BSL is assessed, after which air tamponade of the anterior chamber of the eye is performed in the space between the posterior layer-by-layer graft and BSL. Then the posterior layered graft is centered relative to the previously marked zone of descemetorhexis. In the postoperative period, BSL is removed after the resorption of the air bubble.

EFFECT: invention provides a full-fledged air tamponade of the anterior chamber of the eye, without migration of air or gas-air mixture into the vitreous cavity, in case of violation of the integrity of the recipient's iris lens diaphragm.
