FIELD: medicine; surgical dentistry.
SUBSTANCE: after the functional diagnostics of the dentoalveolar system, virtual modeling of the orthopedic structure is carried out, taking into account the individual trajectories of the temporomandibular joint. The virtual installation of dental implants is carried out taking into account the virtually simulated prosthetic structure. A collapsible navigation surgical template is virtually designed, including a template for forming holes for installing pins supported by teeth, a base for a collapsible navigation surgical template with lodgements, a template for positioning the base of a collapsible navigation surgical template, a template for forming beds and installing dental implants with mating parts for lodgements. They are printed on a 3D printer. Structural changes to the orthopedic structure, including mating parts for lodgements and holes for titanium bases are virtually made and manufactured on a milling cutter. The patient is given infiltration or general anesthesia for medical reasons. Further, using a template for forming holes for the installation of pins, holes are formed in the bone tissue. Then this template is removed, the base of the collapsible navigational surgical template with lodgements is positioned using the template for positioning the base. After that, this template is removed and a low-traumatic extraction of teeth is performed. To the base of the collapsible navigational surgical template with lodgements, with the help of mating parts for lodgements, a template is fixed for forming lodges and installing dental implants. Bone tissue is drilled through the gum in order to prepare the beds and dental implants are installed. This template is removed and transmucosal abutments are fixed to the installed dental implants, to which titanium bases are fixed with screws. The orthopedic structure is positioned to the base with lodgements using the mating parts for the lodgements. Next, the orthopedic structure is fixed to the titanium bases with the help of a fluid light-curing material. Then the screws of the titanium bases are unscrewed and the orthopedic structure is removed from the patient's oral cavity. Next, the mates for the lodgments are removed, the final grinding and polishing of the orthopedic structure is performed. After that, the base of the collapsible navigational surgical template with lodgements is removed from the oral cavity. The prosthesis is then fixed to the transmucosal abutments with screws.
EFFECT: method makes it possible to restore chewing function with minimal trauma in one day and obtain a high aesthetic result while maintaining nature-like aesthetics.
