FIELD: medicine; ophthalmology.
SUBSTANCE: preliminary, ultrasound B-scan, ultrasound biomicroscopy and wide-field optical coherence tomography are performed to detect and localize vitreoretinal fusion. If it is detected in the region of the extreme periphery of the retina, the treatment is carried out on the OptiMedica Pascal Photocoagulator laser unit with a wavelength of 532 nm, for which a Goldman contact lens is placed on the eye. Prophylactic laser coagulation of the retina (RLC) is performed by applying a quadratic pattern of 3*3 or 5*5 dots to the area of vitreoretinal fusion at a distance of two diameters from each other, completely covering the area of vitreoretinal fusion and going beyond its limits by 2–3 rows, with power of 80–250 mW, spot diameter of 200 µm, exposure of 0.03–0.05 s. When a vitreoretinal fusion is detected in the region of the middle periphery of the retina, a color photograph of the fundus is performed on a Navilas 577s navigation laser unit. After that, a treatment plan is applied to the obtained image in a form of a scheme of patterns in a checkerboard pattern at a distance of two diameters from each other, completely covering the vitreoretinal fusion zone and going beyond it by 2–3 rows. Then, using the eraser function, the treatment plan is adjusted without affecting the surface of the vessels. After that, prophylactic LCS is carried out when coagulates are applied to selected areas of the fundus with a power of 80–160 mW, an exposure of 0.03–0.05 s, a spot diameter of 200 μm and a wavelength of 577 nm.
EFFECT: method makes it possible to strengthen and stabilize the retina, eliminate its elevation in the area of vitreoretinal fusion, reduce the intensity of exposure to laser energy, and reduce the risk of complications.
