FIELD: medicine.

SUBSTANCE: group of inventions relates to means and methods used in fetal surgery. A balloon is inserted into the fetal trachea through the oral cavity on a conductor to the tracheal bifurcation zone under ultrasound control. The conductor is made of two separable parts — short and long. Inside the conductor, there is a cable inserted as a frame made with a diameter to enter freely inside both the long and short parts of the conductor. After the introduction of the conductor, the cable is removed from the conductor. Physiological solution is injected into the conductor, inflating the balloon with it and blocking the entrance to the bronchi. Then a pusher cable is inserted into the conductor. In the process of removing the conductor from the fetal mouth, the short part is separated from the long one so that the short part of the conductor with the inflated balloon remains inside the trachea of the fetus, and the tip of the short part of the catheter protrudes from the fetal oral cavity. Then the long part of the conductor is removed, removing it from the amniotic cavity. After delivery, the tip of the short part of the balloon protruding from the fetal oral cavity is removed from the trachea. The method is implemented using a device for intrauterine occlusion of the fetal trachea in congenital diaphragmatic hernia and fetal lung hypoplasia. The device is made of a flexible elastic material. It has a conductor with a channel for the introduction of saline into the cuff of an inflatable balloon located at the distal end. The conductor consists of two parts — short and long with the possibility of separating the short part together with the balloon from the long one. At the same time, a non-return valve is located at the entrance to the inflatable balloon. The short part of the conductor is made with the possibility to protrude from the oral cavity of the fetus. The device contains a removable device separating the long and short ends of the conductor, which is made in the form of a pusher cable. The device additionally contains a cable made with such a diameter to move inside both the long and short parts of the conductor and serve as a frame when it is inserted.

EFFECT: group of inventions allows to expand the arsenal of technical means for intrauterine balloon occlusion of the trachea in a fetus with congenital diaphragmatic hernia and lung hypoplasia, to ensure the stability of the guidewire insertion into the fetal trachea through the oral cavity under ultrasound control and the stability of separating the short part from the long one, to avoid the need for repeated fetoscopy and bronchoscopy procedures, reduce the risks to the fetus and woman, improve fetal survival.
