SUBSTANCE: invention can be used for the treatment of avascular necrosis of the femoral head. Arthroscopic diagnostics of the pathological process is carried out from the side of the joint cavity with obtaining an image. Staging is determined by intra-articular changes in avascular necrosis of the femoral head, as: Stage 0 — no pronounced changes; Stage I — initial degenerative changes in anatomically significant elements, the formation of chondroid bodies and signs of synovitis; Stage II — cartilage deformation in the projection of the localization zone of the femoral head cyst, with defects, osteochondral fragments, signs of an impression fracture; Stage III — deformity of the femoral head and the development of secondary coxarthrosis. When stage 0 is detected, conservative treatment is carried out aimed at decompression of the femoral head; when stage I is detected, a revision and sanitation of the hip joint, partial capsulotomy in combination with tunnelization, bone grafting or osteotomy are carried out; when stage II is detected, an audit of the joint cavity is carried out, sanitation of the hip joint, partial capsulotomy, synovectomy in combination with osteotomy; when stage III is detected, hip arthroplasty is performed.
