FIELD: medicine.
SUBSTANCE: invention relates to clinical laboratory diagnostics, and can be used to assess the condition of a patient with multiple organ failure syndrome. Blood pressure, heart rate, respiratory function indicator, hemoglobin, blood plasma creatinine, blood glucose, leukocytes, total blood bilirubin levels, and the body temperature are assessed. Additionally, the level of lymphocytes and total blood protein are determined, and systolic blood pressure is recorded as blood pressure. Measurements are carried out at least 2 times with an interval of 12–72 hours. Based on the data obtained, the severity coefficient of the patient’s condition with multiple organ failure is determined using the proposed formula:
G={√(Bl/20)+0.37×(T-36.6)+1.3×((Ps×V-1200)/
1200)+WBC/7.5×109+0.5×LYM/2.9×109+√(K/60)
+D+60/OB+140/HB+(140/SAD)2+(GLU/6,5)2}-8.6,
where G is the severity coefficient of the condition;
Bl is the level of total blood bilirubin, µmol/l;
T is body temperature:°C;
Ps is the heart rate, number of beats/min;
V is the patient’s age, full years;
WBC is white blood cell count,×109/l;
LYM is the number of blood lymphocytes,×109/l;
K is blood creatinine level, µmol/l;
D is the coefficient of respiratory function determined as (d/16)2, where d is the frequency of the patient’s respiratory movements in 1 minute, or is taken equal to 40 when carrying out prolonged mechanical ventilation;
OB is the level of total blood protein, g/l;
HB is blood hemoglobin level, g/l;
SAD is systolic blood pressure, mm Hg;
GLU is blood glucose level, mmol/l. If an increase in the value of the G coefficient by 1 or more is detected, the deterioration of the patient’s condition is determined. If a decrease in the G coefficient by 1 or more is detected, an improvement in the patient's condition is assessed.
EFFECT: method provides the possibility to increase the accuracy of assessing the severity of the condition, ensuring the receipt of assessment indicators in the early stages, a more complete assessment of systems and the body by determining the severity of violations of their functions, accelerating the process of obtaining an objective assessment of the severity of the condition based on a limited number of the above-mentioned indicators, as well as providing the possibility of dynamic control during treatment.
