METHOD OF TREATING KERATOCONUS, INCLUDING IN PATIENTS WITH CORNEAL THICKNESS OF LESS THAN 450 MICRONS, USING ULTRAVIOLET CROSS-LINKING OF CORNEAL COLLAGEN IN COMBINATION WITH PERSONALIZED TRANSEPITHELIAL PHOTOREFRACTIVE KERATECTOMY Russian patent published in 2024 - IPC A61F9/08 A61N5/06 A61K31/525 A61P27/02

Abstract RU 2814093 C1

FIELD: medicine; ophthalmology.

SUBSTANCE: invention can be used for the treatment of keratoconus of stages I–II, including patients with a corneal thickness of less than 450 microns. To do this, the cornea is de-epithelialized with laser phototherapeutic keratectomy using a Microscan Visum excimer laser. A personalized ablation algorithm is calculated using the Microscan Visum excimer laser with Kerascan software. Select the type of operation “High Order Aberrations along the Corneal Wave Front”, the diameter of the optical zone is 4.0–4.5 mm, the diameter of the ablation zone is 7.9–8.9 mm. Data from the patient’s subjective correction are entered into the columns “sph”, “cyl” and “ах”. The ablation depth is adjusted using the “incomplete ablation” parameter from 100 to 33%. After laser ablation, the stroma is saturated with a 0.25% hypoosmolar solution of riboflavin for 30 minutes. Then ultraviolet irradiation is carried out with the following parameters: wavelength of 370 nm, irradiation power of 3 mW/cm2. 6 sets of 5 minutes are performed each with instillation of a riboflavin solution between sets. The operation is completed by applying a soft contact lens.

EFFECT: invention provides a more spherical cornea, improved keratometry data, improved corneal ectasia indices, increased visual acuity without losing the effectiveness of corneal strengthening.
