METHOD FOR TWO-STAGE SURGICAL CORRECTION OF EXTENSIVE TRACHEO-LARYNGEAL DEFECT COMBINED WITH TRACHEOMALACIA IN THORACIC TRACHEA Russian patent published in 2024 - IPC A61B17/00 A61F2/20 

FIELD: medicine.

SUBSTANCE: when implementing the method for two-stage surgical correction of an extensive tracheo-laryngeal defect in combination with tracheomalacia in a thoracic trachea, implants are used in the form of arc-shaped curved plates of porous-permeable titanium nickelide with thickness of 0.7–0.8 mm, porosity of 30–60%, pore size of 200–1000 mcm, surrounded by two layers of jersey knitted from nickel-titanium wire with thickness of 28–30 mcm and formed in the form of a flap or stocking. At the first stage of the method, a tracheomalacia section is localized, a wound incision is formed along a tracheal wall and a skin incision at the level of the larynx wall, and the tracheomalacia section is reinforced with the above implant. Then, said implant is placed in the pre-formed wound canal in a projection of the tracheal wall, skin incisions are closed and a T-shaped stent is placed. At the second stage, after the formation of the airway lumen sufficient for breathing with the preserved defect of the anterior wall of the larynx and trachea, a T-shaped stent is removed, skin autologous flap is formed on a pedicle from adjacent tissues and sutured into the defect with the skin inside. Above implants of the required shape and size are formed from the above porous plates and jersey, which are placed transversely to the skin flap, reinforced with support on the lateral walls of the defect, the edges of the skin wound are closed.

EFFECT: invention provides higher reliability of implant fixation and efficiency of surgery for plastic replacement of cartilage defects of tracheo-laryngeal segment.
