SUBSTANCE: invention relates to medicine, namely to traumatology and orthopaedics, and can be used in surgical treatment of intra-articular fractures of posterior tibia plateau. In the popliteal region, an angular incision is made with an angle open to the outside. Fascia is exposed, which is dissected longitudinally, inwards from a medial cutaneous nerve and a small saphenous vein. Internal head of the gastrocnemius muscle is retracted inward and the popliteal neurovascular bundle is identified. Popliteal and soleus muscles are cut off from the inner edge of the tibia, a periosteal-muscular flap is formed, which is taken outwards, covering the popliteal neurovascular bundle.

EFFECT: method enables minimizing the traumatism of surgical treatment of the intra-articular fractures of the posterior tibia plateau by performing the surgical approach which does not require the extraction of the neurovascular bundle, providing sufficient view and manipulation space for reduction and fixation of fragments with fractures of posterior parts of tibial plateau, due to absence of need to cut off tendons of heads of gastrocnemius muscle, as well as by providing soft-tissue protection of the popliteal neurovascular bundle during surgical manipulations.
