FIELD: medicine; ophthalmology.

SUBSTANCE: method involves simultaneous posterior evisceration or enucleation of eyeball and transcutaneous or transconjunctival subperiosteal orbitotomy using endoscopic optics with diameter of 4 mm and a viewing angle of 0°, as well as a suction-raspatory. Then the periosteum is dissected from the bone along the edge of the fracture of the inferior wall of the orbit. Plasty is performed by placing a modeled defect of the lower wall of the eye socket with overlapping of the fracture edges of the implant made of porous polytetrafluoroethylene with thickness of 1 mm. Then the orbitotomy wound is closed in layers. An orbital insert of porous polytetrafluoroethylene with diameter of 20 mm is implanted into a fibrous capsule of the eye after the performed evisceration or into a muscular funnel with hemming of rectus muscles in places of their attachment to sclera in horizontal and vertical planes to orbital insert after enucleation. A teno-conjunctival coating is performed and a conformer is inserted into the conjunctival cavity to prevent conjunctival retraction. In the postoperative period, after reducing conjunctival edema, individual ocular prosthetics is performed.

EFFECT: method makes it possible to increase efficiency of rehabilitation of patients after severe injury of eyeball and orbit in case of combat gunshot injury, to reduce length of treatment and rehabilitation of patients.
