FIELD: medicine; gynecology.

SUBSTANCE: invention relates to treatment of patients with HPV-associated squamous intraepithelial lesions CIN II–III of uterine neck. Radio wave conization of uterine cervix is performed using a sail electrode of the Surgitron apparatus in the "Incision and coagulation" mode with removal of pathological changes of a precancerous and tumor nature. After removal of cervical tissue, intraoperative photodynamic therapy is carried out: on the surface of the surgical wound and around the wound, at distance of 0.5–1 cm, and also a photosensitizer "RadaGel" with an active substance Radachlorin 0.5% is applied on the cervical canal, exposure time is 60 min. After that, irradiation is performed in continuous mode of the whole area with the help of phototherapeutic light-emitting diode apparatus, with a wavelength of optical radiation of 660 nm, energy density of 150–250 J/cm2, for 10–18 minutes, wherein the duration of exposure to achieve the maximum therapeutic effect is calculated by the following formula: T=Ds/Ps, where T (s) is the light exposure time, Ds (J/cm2) is the dose density of the supplied radiation, Ps (W/cm2) is the power density at the end of the light guide.

EFFECT: proposed method provides higher clinical effectiveness, reduced rate of recurrences, avoided additional resection in case of positive histological margin, as well as improved patient’s quality of life.
