METHOD OF TREATING MASSIVE RUPTURES OF ROTATOR CUFF TENDONS Russian patent published in 2024 - IPC A61B17/00

SUBSTANCE: invention refers to medicine, namely to traumatology and orthopaedics, and can be used for treating massive ruptures of rotator cuff tendons. Method involves approaching the shoulder joint, visualizing and assessing the degree of damage and retraction of rotator cuff tendons. From a separate approach in the lower one-third of the shin, an autograft tendon of a peroneal longus muscle is taken in the amount of ½ of its width. After assessing the degree of retraction of the injured tendons, an insertional site of the humeral head is prepared. Both ends of the autograft tendon are sutured with non-absorbable sutures. One end of the autograft tendon is delivered through the edge of the retracted tendon, and the other end is fixed transosseously on the insertion platform through the greater tubercle of the humerus with non-absorbable sutures. Thereafter, the end of the autograft delivered through the retracted tendon is stretched and similarly fixed on the insertional site next to the place of the previous fixation. Then the free end of the autograft tendon is repeatedly brought through the retracted tendon and laid on the head of the humerus directly near its previous application and also transosseously fixed with non-absorbable sutures. This technique is repeated until the head of humerus is completely covered. Then, superimposed "loops" of the tendon autograft are fixed to each other by an absorbable suture, single sutures.

EFFECT: method provides higher clinical effectiveness in massive rupture of rotator cuff tendons ensured by recovery of shoulder joint function and pain management.
