FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to surgery, traumatology and orthopaedics, and can be used for treating patients with stenosing ligamentitis of A1 annular ligament, involving dissection of the ligament under ultrasound control using an instrument with a cutting element at the end. Linear skin incision 3 mm long is performed along distal palmar fold, soft tissues are bluntly separated, annular ligament A1 and tendons of deep and superficial flexors of hand are verified under ultrasound control. Ligamentotome is brought into the incision, the handle of which is connected by means of a conductor bent at angle of 150 degrees to a working part formed by a guide and a stop, connected to each other by a semi-moon cutting element so that the guide projecting forward is wider than the cutting part and the stop. Ligamentotome is placed parallel to the tendons and rests with a semilunar cutting element against the edge of the annular ligament, dissecting it in one motion, advancing the ligament distally under ultrasound control. Instrument is removed, the ligament dissection is monitored for ultrasound, the skin incision is closed with a bandage.
EFFECT: method provides reduced traumatisation of adjacent tissues in the wound and tendons during dissection of the annular ligament due to the peculiarities of the approach and the shape of the used ligamentotome.
