METHOD OF HEMODYNAMIC PLACENTAL INSUFFICIENCY DIAGNOSTICS AT EARLY STAGE Russian patent published in 2008 - IPC A61B8/06

FIELD: medicine; obstetrics.

SUBSTANCE: ultrasonic Doppler examination is performed to determine blood velocity in umbilical cord artery and right uterine artery to get initial indices of systolic-diastolic ratio in umbilical cord artery (SDRuca) and systolic-diastolic ratio in right uterine artery (SDRua). Placental index (PI) is calculated by formula: PI=1/( SDRuca x SDRua), where: PI is placental index; SDRuca is systolic-diastolic ratio in umbilical cord artery; SDRua is systolic-diastolic ratio in right uterine artery. It is combined with hyperventilation load test with inspiration within 2 sec and expiration within 4 sec under metronome control. In 30 sec after test beginning Doppler examination of is performed again to determine blood velocity in umbilical cord artery and right uterine artery to get initial indices of load systolic-diastolic ratio in umbilical cord artery (SDRucaload) and load systolic-diastolic ratio in right uterine artery (SDRuaload). Load placental index (PIload) is calculated by formula: PIload=1/(SDRucaloadxSDRuaload), where: PIload is load placental index; SDRucaload is load systolic-diastolic ratio in umbilical cord artery; SDRuaload is load systolic-diastolic ratio in right uterine artery. Standardized placental index (PIST) is calculated by formula: PIST=PI/P, where: PIST is standardized placental index; PC is placental index; P is proper value of PI for month of pregnancy at examination. Rate of placental index load shift (ΔPI) is calculated by formula: ΔPI= PIload/PI, where: ΔPI is rate of placental index load shift, PIload - load placental index, PI is placental index. Value of PIST equal or more 1.0 and value ΔPI more 1.0 indicates normal placental blood flow with favourable pregnancy termination. Value of PIST equal or more 1.0 and value ΔPI less than 1.0 indicates initial manifestations of hemodynamic placental insufficiency with unfavourable pregnancy termination. If value of PIST is less than 1.0 and value of ΔPI is whatever, hemodynamic placental insufficiency with unfavourable pregnancy termination is diagnosed.

EFFECT: enables to diagnose initial manifestations of hemodynamic placental insufficiency.
