FIELD: medicine.
SUBSTANCE: invention relates to field of medicine, namely to neurosurgery. Resection of anterior-lateral parts of temporal area is performed. Anterior surface of malar arch is skeleted, cut of temporal muscle is continued to zygomatic process of frontal bone, from malar arch muscle flap is separated on small segments in region of zygomatic process of frontal bone and zygomatic process of temporal bone and in said places saw cuts aremade. Temporal muscle with malar arch fragment is separated from bone downward up to infratemporal crest, lateral parts of main bone wings are resected, dura mater is opened around lateral fissure. Subarchnoid membrane of lateral fissure cistern is cut more laterally than surface veins above temporal lobe pole. Deformation of lateral fissure up to temporal bone isthmus is performed. After separation of temporal lobe pole its resection is performed. Inferior horn cavity is opened on medial border of pole resection, choroid plexus is displaced medially, hippocampus is ablated in blocks, simultaneously ablation of (para)hippocampal gyrus is performed and deefferentation of area of (para)hippocampal gyrus hamulus is performed by dissecting medullary substance along the border of gyrus olfactorius to cranial fossa base and in region of isthmus from inferior horn top to olfactory sulcus. After hippocampus resection, resection of almond nucleus is performed.
EFFECT: reduction of traumaticity of operation, which is achieved due to combination of pterional and zygomatic approaches and dissection of arachnoid membrane in area of lateral fissure.
