FIELD: medicine.
SUBSTANCE: visometry, perimetry, ophthalmoscopy, optical coherence tomography (OCT) and neurological examinations are conducted to detect microsymptoms and are followed by electrophysiological examination (EPE). Sudden monocular visual impairment, central visual field loss, red and green coning of vision, and nystagmus are detected in the patient with primary monocular optic neuritis; the OCT shows a mild prominence of the optic disk; the EPE shows the loss of electrical lability of the optic nerve; the neurological examination aims at detecting the microsymptoms presented by a dissociation of symptoms, i.e. reduced or missing abdominal reflexes and increased tendon and periosteal reflexes, intentional tremor accompanying goal directed movements in finger-to-nose and heel-to-shin tests. What is also conducted is the T2 Fat Saturation and T2 STIR magnetic resonant tomography of the orbits in three projections: axial, coronal, oblique sagittal; with a section thickness of no more than 2 mm. If the detected microsymptoms are accompanied with a demyelinating lesion of not less than 3×3 mm found in the orbital segment of the optic disk, monocular optic neuritis at the onset of multiple sclerosis is diagnosed.
EFFECT: technique provides the timely diagnosis of the onset of multiple sclerosis that is expected to enable starting early therapy pathogenetically targeted on the disease progression prevention, remission prolongation, and visual function improvement.
