FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to dentistry, oncology, and can be used in rehabilitation of patients after resection of the affected area of the lower jaw with primary osteosarcoma of the lower jaw. For this, a number of successive steps are carried out. Stage of modeling in the form of constructing the actual 3D stereolithic model of the patient's lower jaw after resection with subsequent alloplasty, and also a 3D stereolithic model of the expected form of the mandible. Stage of corrective surgical intervention includes: gum formation, creation of an individual impression tray, removal of impression from implants, creation of plastic beam construction of a prosthesis with support on implants, manufacturing of a beam prosthesis. During the fitting of the beam prosthesis a bite pattern with occlusal ridges is made. Then, the prosthesis is installed, including screw fixation of the beam prosthesis and individual abutments to the implant and final application of the removable prosthesis. After a repeated standard procedure, its fit is assessed by occlusion, fit and aesthetic characteristics. Throughout the period of prosthetics, mesodieencephalic modulation (MDM) sessions in the amount of 14 are performed in a certain sequence: 9 sessions – from the second day after the implantation of half-burned cylinders, one session per day. During these days, no other manipulations are performed. Then, MDM is administered 1 session per day after each invasive intervention in the oral cavity – only 4-5 sessions. In this case, 1 session of MDM is carried out during the creation of an individual impression tray a day after the impression is removed from the implants. 2 MDM sessions are conducted during the creation of a plastic beam structure: 1 session - during the creation of the framework of the beam prosthesis made of plastic and 1 session - after fitting the beam prosthesis. 1 MDM session is performed during the manufacture of the prosthesis after checking the design of a removable prosthesis in the oral cavity. 1 session of MDM is carried out during the installation of the prosthesis in the mouth of the patient after the establishment of the prosthesis. With MDM pulsed currents with a frequency of 10,000 Hz, modulated in the low-frequency range from 20 to 100 Hz, are used. MDM is carried out through electrodes, which are fixed on the head of the patient by the fronto-occipital technique. Current is selected individually, then the session is carried out on the selected current strength, the duration of the session is 30 minutes.
EFFECT: method ensures minimization of risk of metastasis and relapse of the tumor due to the stabilization of the psychoemotional state of patients, creation of clinically significant local and systemic anti-stress effect; rapid decrease in the severity of the pain syndrome; acceleration recovery function.
