FIELD: medical equipment.

SUBSTANCE: group of inventions refers to medical technology, namely, to an apparatus and method for the safe positioning of a coronary stent in the coronary arteries. Device intended for positioning the coronary stent in the coronary arteries consists of a body with a screw rotation wheel, the upper part of which protrudes above the body, and the lower part is located inside the housing and is made as a single element with a horizontal screw located on the slider with a threaded element for engagement with the screw with the possibility of moving it longitudinally inside the housing. On the axis of the slider with the possibility of rotation around its axis is placed a bush with an elastic tie with the clamps on it made of elastic material. Device has a truncated shape in the front part and recesses adapted to be held by the operator's left hand, and an upper rear part made in the form of a tail for the palm rest of the right hand. In the upper part of the body is a window made of a transparent material, under which a control line is placed inside the case for determining and monitoring the location of the mechanism, fixing the delivery system of the coronary stent inside the device. In the front and back parts of the body are made through holes for placing the delivery system with a coronary stent. In the side opening of the housing there is a fixing button, the inner side of which is located inside the case and is a rectangular plate, centered by stiffeners located inside the body, allowing it to move freely inside the body in the direction of the wedge, located on the opposite side of the slider and consisting of a pressure wedge and a conical wedge. Pressure wedge is located in the side opening of the slider on the side of the fixing button and is a parallelepiped with a rounded side surface on the side of the fixing button, made inside with a through hole formed by a cut on one side and cut at an angle from the other side. Sections are made in such a way that they allow placing a butt-like wedge in it, the lower part of which is cut at an appropriate angle, and the upper part is a cone, protruding from the hole in the slider from the side of the screed and touching the screed. In the pressure and conical wedges are made holes, through which, with an open tie, placed on the slider, moves the delivery system of the coronary stent. In the groove of the side of the slider on the side opposite to the fixing button, a spring is placed in contact with the surface of the pressure wedge at the cutting site. Method for positioning a coronary stent in the coronary arteries, characterized by the fact that when the rotation wheel of the screw, which is made in the form of a single component with a horizontal screw inside the device casing, rotates horizontally in the desired direction of the slider. On the axis of the slider with the help of the bush, with the possibility of rotation around its axis, there is a coupler with fasteners made of elastic material, in the through hole of which, in the open state, the system of coronary stent delivery is placed by sequential introduction of a coronary stent on the delivery system inside the device, for which the operator presses the fixing button, the inner rectangular part of the button presses on the push wedge pushing out the tapered wedge that expands the screed and through the through holes, made in the body, the system of delivery of the coronary stent is placed. At the termination of pressing of the fixing button, the spring pushes out the pressure wedge, thereby returning the cone wedge and button to the initial position, and the screed compresses and holds the delivery system. Operator then manually passes the coronary stent on the delivery system located in the device to the area of the coronary artery lesion. Press the lock button again, the screed is opened and the operator moves the device to the required distance to fix it with the fingers of the left hand, releases the fixation button and with the right hand rotates the wheel of rotation of the screw, made in the form of a single component with a horizontal screw inside the case of the device, moving horizontally in the desired direction of the slider, on the axis of which, with the help of a bush with the possibility of rotation around its axis, there is a screed with fixatives made of elastic material, in the through hole of which, in the open state, the coronary stent delivery system is placed, converting the rotational movement of the rotation wheel into translational movement of the slider, the operator moves the delivery system to the required distance to position the coronary stent inside the vessel. Also, when the button is pressed, the operator can reset the mechanism fixing the delivery system of the coronary stent by rotating the wheel in the required direction, which allows to continue further positioning of the coronary stent forward or back within the size of the ruler, placed on the slider under the transparent case window, to each side.

EFFECT: inventions allow the most accurate, fast and safe positioning of the coronary stent with simple and complex anatomical lesions of the coronary bed, in particular in the coronary artery during endovascular coronary stenting to restore lumen of narrowed as a result of damage artery sites.
