FIELD: medicine.
SUBSTANCE: invention relates to radionuclide diagnostics and transplantation, and can be used for determining mitochondrial myocardial dysfunction after cardiac transplantation using a radionuclide method. Radiopharmaceutical is administered once 99mTc-technetryl. Early and delayed scintigraphic examination is performed 45-60 minutes and 4-6 hours after the radiopharmaceutical introduction to obtain scintigraphic images of 17 myocardial segments. Average count of pulses in each of 17 segments of early and delayed images is determined. Further, the radiopharmaceutical elutriation coefficient (RE) in each segment is determined by formula: RE = ((N1*e-λ⋅t-N2)/(N1*e-λ⋅t)*(4/t))*100. N1 is average pulse count in a specific segment on an early scintigraphic image. N2 is average count of pulses in a specific segment on a delayed scintigraphic image. t is time interval in hours between delayed and early phases of scintigraphic examination. λ is the radiopharmaceutical decay constant. Conclusion on mitochondrial dysfunction is made for each segment when obtaining the value of RE > 17.4±5.2.
EFFECT: method provides higher prognostic value and accuracy of determining the prevalence of mitochondrial myocardial dysfunction and clinical effectiveness for patients after heart transplantation, as well as reducing the radiation exposure of the study by evaluating the early and delayed images with a single administration of radiopharmaceutical 99mTc-MIBI by calculating its washing-out coefficient in each segment of heart.
