FIELD: medicine.
SUBSTANCE: invention refers to medicine and can be used in surgical management of congenital and acquired funnel-shaped chest deformities in children. Two incisions of skin and subcutaneous fat 3–4 cm long along lateral surfaces of the thorax are performed. Substernal tunnels are formed by placing the tunnel in the area of maximum deformation. Tunnel deformation is corrected. Conductor is fixed to the drain tube tunnel, with the help of which correcting C-shaped titanium plates are passed behind the sternum with the concave end, leaving the concave part behind the breastbone. Plates are unfolded thus giving sternum and ribs physiologically correct position. At that, during formation of the first retarded tunnel, tunneler is introduced to the right along the midclavicular line in the intercostal space located above maximal abdominal tenderness. Tunnel is carried out under the greater pectoral muscle towards the midclavicular line on the left, in the intercostal space located below the maximum depression of the sternum. After introduction of the first titanium plate of C-shape and its turn the second tunneling device is introduced similarly to the first one from the left side, at that point of intersection of the second tunneling device and introduced C-shaped plate is located under maximum depression of sternum. Then, the second C-shaped titanium plate is inserted and unfolded, at that, lifting is performed by point of intersection of two C-shaped plates. Plates are fixed to ribs.
EFFECT: invention provides correction of thoracic deformation, reduced traumatism, reduced risk of postoperative complications due to uniform distribution of load on intercostal margins and creation of such position of plates, at which maximum-effective elevation of deep local defect is achieved.
