SUBSTANCE: invention relates to biotechnology, namely to endocrine surgery. Essence of the method: before the surgical treatment of patients with thyroid pathology, pharmacogenetic testing is performed to determine the presence or absence of Val174Ala polymorphism in the human SLCO1B1 gene. When revealing polymorphism in the form of replacement of valine with alanine in the 174th position of the amino acid sequence of the SLCO1B1 protein, leading to an increase in plasma thyroxine sulfate concentration by 24%, it is advisable to choose, all other things being equal, the organ-preserving volume of thyroid surgery. Organ-preserving surgery, which does not require the appointment of maximum dosages of levothyroxine in the postoperative period, will not increase the frequency of adverse drug reactions, especially if they are taken for a long time. If a homozygote is detected, that is, without polymorphism in the form of a valine-to-alanine substitution at the 174th position of the amino acid sequence of the SLCO1B1 protein, it is advisable to choose, all other things being equal, a radical scope of surgical treatment.

EFFECT: use of the method based on pharmacogenetic testing with the detection of the Val174Ala polymorphism in the SLCO1B1 gene makes it possible to predict the frequency of adverse drug reactions in the postoperative period and the possibility of determining the maximum safe dose of levothyroxine sodium in hormone replacement therapy of postoperative hypothyroidism.
