METHOD FOR DIAGNOSING OF ABNORMAL OCCLUSION FOR CHOOSING THE TACTICS OF ORTHOPEDIC TREATMENT Russian patent published in 2021 - IPC A61B6/03 A61C7/00

Abstract RU 2760086 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to dentistry, and can be used for 3D planning of dental treatment and its subsequent implementation using digital 3D technologies. Anomalies in the height of the occlusion and tracing are established using a computer program, in particular the InVivo Anatomage program, by means of which the skeletal point corresponding to the apex of the anterior nasal spine (ANS) is determined, the skeletal point corresponding to the most protruding point of the chin symphysis (Pog), and skeletal points, corresponding projections of the entrance of the lower alveolar nerve into the right branch of the lower jaw (Fm-R) and into the left branch of the lower jaw (and Fm-L) are determined. Then, angles (ANS-FmR-PogSAG, ANS-FmL-PogSAG) are plotted using these points in the projection of the median-sagittal plane, the values of which are compared. In the case of a difference in the angles of the bite height on the left and right by from 0.3 ° to 1.9 ° and in the presence of signs of TMJ dysfunction, treatment with a splint splint with remission of signs of dysfunction is prescribed for at least 3 months, and implantation is performed in the presence of secondary partial adentia. In the case of a difference in the angles of the bite height on the left and right by from 2° to 3.9° in the presence of TMJD dysfunction, therapy is prescribed on a splint-splint until a stable remission is obtained for at least 5-6 months, then orthodontic treatment begins with the elimination of anomalies in the position of the teeth and correction of the occlusal plane with the use of mini-screws and surgical treatment with implants, orthopedic treatment with the installation of artificial crowns on implants, taking into account the correction of the bite height and the occlusal plane. In the case of a difference in the bite height of more than 4°, pre-orthodontic treatment is carried out on a splint-splint for up to 12 months with remission of at least 6 months, while the stage of orthodontic treatment is carried out on a bracket system, for the correction of extrusion and mesial displacement, the locks of the bracket system are fitted with overcorrection , for corpus correction of teeth and elimination of extrusions, mini-implants are additionally used; when creating a place for dental implantation, opening springs are used in combination with mini-implants in the retromolar region to create additional anchorage. After creating a site, dental implantation is performed, permanent prosthetics on implants with metal-ceramic crowns are performed, taking into account the correction of the occlusion height and the occlusal plane.

EFFECT: method provides an increase in the accuracy and quality of the diagnosis of anomalies in the height of the bite and the choice of tactics of orthopedic treatment, excluding measures for reprosthetics due to the rapid identification of all necessary points on the skull without reference to soft tissues, in the personification of the data obtained in the manufacture of prosthetic structures by a dental technician and in reducing the risk of dysfunction temporomandibular joint.
