SUBSTANCE: invention relates to medicine, namely to neurosurgery, neurology, and can be used for intraoperative localization of conduction pathways of speech zones during surgery with awakening in patients with formations of the hemisphere dominant in speech. Intraoperative awakening, speech testing, cortical brain mapping for localization of speech zones during neurostimulation with simultaneous performance of speech tasks by the patient are carried out. Neurostimulation of the conductive pathways of the white matter is performed with a monopolar electrode at a stimulus strength of 8 mA in cathodic polarity with a frequency of 60 Hz and a duration of 4 seconds. If there are no errors in the patient's performance of speech tasks, resection is continued in this zone, then stimulation is repeated, if speech disorders occur, the stimulus strength is reduced sequentially in steps of 1 mA, and the resection is stopped while maintaining errors at a stimulation strength of 2.5 mA.
EFFECT: use of the invention improves the accuracy of the verification of speech areas due to the localization of the conductive paths of the white matter, uniting the speech areas into a single network, which reduces the likelihood of developing a permanent neurological deficit in the postoperative period.
