FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to traumatology, orthopedics and neurosurgery, and can be used for endoscopic decompression of the brachial plexus. The method is performed in 3 stages, which are carried out in the position of the patient "beach chair". The first stage includes the marking of anatomical landmarks for endoscopic ports. In accordance with figure 1 the first point in the area of the rotator interval is marked, the point of execution of the subcoracoid port is marked at a distance of 2-4 cm distally and 2-4 cm medially from the first mark for the introduction of the arthroscope. Removal of tissues of the rotator interval, release of tissues in the area of the coracoid process and its visualization are performed. At the second stage, the arthroscope is transferred to the anterior port in the region of the rotator interval. Under the control of the arthroscope, a subcoracoid port is formed in accordance with the markings made at the first stage. Tissue release and visualization of the pectoralis minor is performed. The pectoralis minor muscle is cut off from the medial edge of the coracoid process, eliminating the compression of the plexus from compression by the muscular abdomen. At the third stage, the arthroscope is transferred to the subcranial port. Under the control of the arthroscope, a mid-clavicular port for the working instrument is performed in accordance with the markings made at the first stage. Then, with the help of an ablator, a shaver, and an obturator, release and separation of tissues in the pectoralis minor muscle cut-off area is performed, and the brachial plexus bundles and the axillary artery are visualized. Then the adhesions and scar tissue between the nerve bundles are divided longitudinally.
EFFECT: method provides simplification of surgical technique, reduction of operation time, reduction of soft tissue trauma due to marking of anatomical landmarks and places of endoscopic ports.
