FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to traumatology, orthopaedics and neurosurgery, and can be used for extra-articular endoscopic neurolysis of brachial plexus. First stage includes marking of anatomical landmarks and projection of apex of coracoid process, marking of the anterior-external port, which is located 1.5–2 cm laterally from the projection of the apex of the coracoid process, through which the arthroscope is inserted, marking an anterior-internal port, which is located 1.5–2 cm inwards from a projection of an apex of a coracoid process, through which a working instrument, an ablator and a shaver are introduced. Further, an anterior-external approach is performed, through which an arthroscope is inserted, an anterior-internal approach, through which a working instrument is inserted, during the approach, only the skin is pierced with a scalpel, and the endoscopic canal itself is made through a puncture by means of a direct clamp of the “mosquito” type towards an apex of the coracoid process. During the second stage, the tissues are released within the coracoid process and the pectoralis minor muscle is visualized. Further, the small pectoral muscle is cut off from the medial edge of the coracoid process, then the tissues are released in the region of the lower surface of the clavicle, subclavian muscle is visualized, its lateral portion is dissected from clavicle to form an access window to brachial plexus. At the third stage, using an ablator, a shaver and an obturator, cicatricial and adhesive tissues are released and dissected around the brachial plexus and between nerve bundles, bundles of brachial plexus, suprascapular nerve and subclavian artery are visualized, cicatricial tissues are dissected by ablator, obturator and shaver, releasing components of brachial plexus from compression.
EFFECT: method provides faster and easier surgical intervention, reduced perioperative blood loss due to specific features of cicatricial tissue dissection.
