FIELD: medicine; traumatology; orthopedics; neurosurgery.
SUBSTANCE: invention can be used for endoscopic decompression and revision of the brachial plexus in the region of the thoracic aperture. At the first stage, anatomical landmarks and arthroscopic ports are marked. Then, therapeutic and diagnostic arthroscopy of the shoulder joint, removal of tissues of the rotator interval, release of tissues in the area of the coracoid process, and formation of the anterolateral port are performed. At the second stage, the coracoid process is visualized through the anterolateral port, and tissue is released in the area of the coracoid process through the anterior port, pectoralis minor is cut off from the medial edge of the coracoid process. At the third stage, tissues are dissected at the medial edge of the base of the coracoid process, the lateral portion of the subclavian muscle is partially cut off from the clavicle and a "window" is formed, which is an access to the thoracic aperture and plexus trunks. At the fourth stage, release and separation of tissues in the region of the thoracic aperture are performed, scar tissues are dissected between the lower edge of the clavicle and the neurovascular bundle, adhesions and scar tissues are separated between the nerve trunks, releasing the components of the brachial plexus.
EFFECT: method allows to speed up and simplify surgical intervention, reduce perioperative blood loss, perform decompression of the brachial plexus through the use of two endoscopic ports.
