FIELD: medicine; traumatology; orthopaedics.
SUBSTANCE: method for endoscopic decompression and revision of the brachial plexus in the area of the interstitial space includes 3 stages when the patient is in the position of "beach chair". At the first stage, anatomical landmarks of the anterior supraclavicular port are marked in the area of the supraclavicular fossa, 1–2 cm behind the middle of the clavicle, and the landmarks of the supraclavicular posterior port are marked, which is located 5–7 cm posterior to the middle of the clavicle. The posterior supraclavicular port is performed, through which the endoscope is inserted, and the anterior supraclavicular port is formed, through which the working instrument is inserted. At the second stage, in the area of the supraclavicular space, using a shaver, an ablator, and an obturator, the adhesive tissues are released in the proximal direction along the suprascapular nerve with dissection of the adhesive and fatty tissues and visualization of the trunks of the brachial plexus. At the third stage, the upper, middle, lower trunks of the brachial plexus, the subclavian artery, and the middle scalene muscle are visualized in the area of the interscalene space. With the help of longitudinal movements relative to the neurovascular bundle, by the obturator, shaver and ablator perform the release and separation of adhesions and scar tissues in the area of the interscalene space.
EFFECT: simplification of surgical technique, reduction of operation time, reduction of soft tissue traumatization due to the use of additional endoscopic ports and staging of endoscopic intervention.
