SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics and neurosurgery, and can be used for endoscopic neurolysis of the sciatic nerve. The method includes 4 stages carried out with the patient in the prone position. At the first stage, palpation is performed to determine the location of the apex of the ischial tuberosity, then anatomical landmarks are marked. An indentation is made 2-4 cm distally and 2-4 cm laterally from the top of the ischial tuberosity to perform a posteromedial port through which the arthroscope is inserted, and then 4-8 cm are retreated laterally and a point is marked for performing a posterolateral port through which a working instrument is inserted. At the second stage, a posteromedial port channel is formed through a skin puncture towards the top of the ischial tuberosity. An arthroscope shaft with an obturator is inserted into the canal, with the tip of which the tip of the ischial tuberosity is palpated. Then, with the help of movements of the obturator in the horizontal plane, a cavity is formed. Further, according to the marking of anatomical landmarks, a posterolateral port is formed in the direction of the ischial tuberosity. At the third stage, the main anatomical landmark is determined - the ischial tuberosity with the united tendon attached to it. The tissues are released outward from the ischial tuberosity at a distance of 4-6 cm. At the fourth stage, the sciatic nerve is released from the surrounding scar tissue using an obturator, ablator and shaver by dilution and dissection of the adhesions along the axis of the nerve, completely freeing it.

EFFECT: method provides for the expansion of the arsenal of technical means by simplifying the surgical technique, reducing the operation time, and reducing soft tissue trauma.
