METHOD FOR INJECTION ELECTROPHORESIS WITH ULTRASOUND GUIDANCE IN THE SUPRAPUBIC REGION AND GUIDANCE IN THE PROJECTION AREA OF THE LARGE SCIATIC OPENING OF THE INJECTION NEEDLE IN THE TREATMENT OF INFLAMMATORY INFILTRATIVE PELVIC FORMATIONS IN MALES Russian patent published in 2022 - IPC A61N1/30 A61B8/08 A61K31/573 A61K31/245 A61K31/4152 A61K38/47 A61K33/14 A61P13/08

Abstract RU 2780163 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely urology, and can be used for injection electrophoresis with ultrasound guidance in the suprapubic region and guidance in the projection area of the large sciatic opening of the injection needle in the treatment of inflammatory infiltrative pelvic formations in males. The patient is placed on his stomach or on the side. Ultrasound examination is performed with a moderately filled bladder. A rectal finger examination and finger compression of the detected formations is performed; in this case, the ultrasound sensor is located in the suprapubic region in the projection of the vas deferens when accessing the lower sections of the vas deferens or in the projection of the large sciatic opening when scanning the upper sections in the projection of the vas deferens. After the image of the formation appears on the ultrasound monitor screen, the needle is immersed under the control of a tracer to the site of infiltrative formation, while a needle with pencil sharpening and a side hole for the introduction of a medicinal substance is used for the administration of drugs. After the needle is immersed in the area of the pathological focus, the drug is administered in the following order: ¼ of the total volume of the drug prepared in the syringe for simultaneous administration, which is 2.5-3.0 ml, is injected through a cannula in the upper part of the needle with a syringe, then the needle is rotated 90° and the second next quarter of the total volume of the drug is injected, then the needle is turned 90° again and the third part of the drug is injected, and the last quarter of the volume of the drug is injected after the third rotation of the needle by 90°. Above the injection site, an electric plate-electrode with the electrostatic charge that the injected drug has is applied to the skin surface in the suprapubic region or in the gluteal region in the projection of the upper sciatic notch, and a plate with the opposite electric pole is applied to the opposite skin surface so that the pathological focus with the injected drug is between two electric plates-electrodes with opposite electrical poles. After that, the electrophoresis apparatus is turned on and an electric field with a force of 15-20 mA is created, with an exposure time of 5-10 minutes. In the initial phases of inflammation, hydrocortisone, 125 mg, is used in dilution of 5.0 ml of 0.9% sodium chloride solution; in the initial phases of the inflammatory process with pain syndrome, analgin 50%, 1.0 ml per 5.0 ml of 0.9% sodium chloride solution is used; in the proliferative and productive phase of inflammation, Longidaza is used at a dosage of 3000 IU in dilution per 5 ml of either 0.9% sodium chloride solution or 0.5% novocaine solution, while the polarity of novocaine coincides with Longidaza, having a sign (+).

EFFECT: method provides treatment of infiltrative pelvic formations by ultrasound scanning through the gluteal region by bimanual compression.
