FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to ophthalmology. Combined treatment of refractory glaucoma is carried out. In the micropulse mode of exposure, the tip is carried out 3.0 mm from the limb, along the upper and then lower hemisphere, avoiding zone of 3 and 9 o'clock, as well as the area of previous antiglaucoma operations (AGO), the following parameters are set: wavelength 810 nm, power 2,000 mJ, exposure time 80 s, cycle length 31.3%. Exposure is performed by sliding movements, with compression, perpendicular to the eye surface. Then cyclophotocoagulation (CPC) is performed in a continuous mode in the lower hemisphere, at 2.0 mm from the limb, avoiding zone of 3 and 9 o'clock, as well as the zone of preceding AGO, with the following parameters: wavelength 810 nm, power 800 mW, exposure time 5 s, number of applications 10, total exposure on area from 90 to 180°. In a particular case, if there is no “click” sound at the first exposure, the power is increased by 250 mW until a characteristic click appears, but not more than 2,000 mW.
EFFECT: method provides a long-term hypotensive effect with minimal complications due to potentiation of the action on the ciliary body under the effect of the micropulse CPC mode and the continuous mode of the laser CPC by improving the outflow of intraocular fluid due to activation of uveoscleral pathway, reduced production of intraocular fluid due to thermal effect on ciliary body.
