FIELD: medicine; preventive and rehabilitation medicine; occupational health.
SUBSTANCE: invention can be used to predict the risk of developing respiratory diseases in workers involved in the production of synthetic detergents. A medical examination of workers involved in the production of synthetic detergents, with an experience of 5 years or more is conducted by determining diagnostic laboratory and functional indicators. Laboratory indicators include the following: the level of eosinophils in peripheral blood, eosinophilic-lymphocyte index (ELI), the level of immunoglobulin E total (IgE), the level of interleukin-6 (IL-6); immunoglobulin A (IgA), content of CD3+CD95+ - relative lymphocytes; the level of tumor necrosis factor (TNF) and the level of very low density lipoprotein cholesterol (CHLLD), the number of eosinophils in a smear from the nasal mucosa, by taking a rhinocytogram. Diagnostic functional indicators include the functional activity of the endothelium determined by ultrasound examination of the vasomotor function of the endothelium on the brachial artery. For those workers involved in the production of synthetic detergents who have the following levels of indicators: the level of eosinophils in the peripheral blood is 6.0% or more, the eosinophil-lymphocyte index is 0.05 or more, the level of total IgE is 180 IU/cm3 or more, the level of IL-6 is 12 pg/ml or more, IgA level is 3.5 g/dm3 or more, TNF level is 9.0 pg/ml or more, CHLDL level is 0.9 mmol/l or more, CD3+CD95+ level of relative lymphocytes is 29% or less, with a simultaneous content of eosinophils in a smear from the nasal mucosa of 11% or more, as well as in the presence of a violation of the functional activity of the endothelium determined by vasoconstriction or expansion of the brachial artery as less than 10% of the initial value in a post-occlusive test with endothelium-dependent vasodilation of the brachial artery, the prognosis of development of respiratory diseases is evaluated as high.
EFFECT: method makes it possible to predict the risk of developing respiratory diseases in workers with an experience of 5 years or more, involved in the production of synthetic detergents, at the stage of premorbid changes, due to the development of a set of objective prognostic criteria being laboratory and functional indicators.
