FIELD: medicine, orthopedic dentistry, orthodontics and gnathology.
SUBSTANCE: invention intended to determine the optimal position of the lower jaw in patients with partial loss of teeth and dysfunction of the temporomandibular joint. The patient is examined, an intraoral (intraoral) scan is performed, control and diagnostic models of the jaws are made, the patient is prescribed to use a hydrostatic mouth guard for a period of 2 weeks, after which the position of the lower jaw is recorded. The patient is sent to a cone-beam computed tomography scan of the skull with a registry, then the scans of the jaws are combined with a cone-beam computed tomography of the skull, and the position of the TMJ heads in the sagittal direction is analyzed in the Arthro-Expert software. If a discrepancy with the norm is detected, the program calculates the required amount of changes in the position of the lower jaw in the sagittal direction (in mm) to achieve normal parameters of the joint space. Next, a mouth guard is performed in the articulator, taking into account the recommendations of the program to achieve normal joint space parameters. The manufactured mouthguard that separates the dentition positions the lower jaw in the sagittal direction, and in the transversal direction the jaw occupies the position that was determined by the hydrostatic mouthguard. To determine the position of the mandible in the vertical direction, the vertical Shimbachi index (LVI) is used. A mouthguard that separates the dentition is made taking into account the type of defect in the dentition with the latter being replaced with artificial teeth (a mouthguard prosthesis).
EFFECT: by taking into account the muscular and articular component of the dentofacial apparatus of patients with partial loss of teeth and dysfunction of the temporomandibular joint, the method allows to determine the central position of the lower jaw in patients of this group.
