FIELD: medicine, dentistry.
SUBSTANCE: invention is intended for use in the diagnosis of patients with dentofacial anomalies associated with TMJ disorders. Clinical, functional and radiation examinations are carried out, during which cone-beam computed tomography of the skull, magnetic resonance imaging of the TMJ, and kinesiography are performed. The anthropometry of teeth, dentitions, apical bases of the jaws on digital models of the dentition is implemented in the Ortho3D program performing a cone-beam computed tomography of the skull, multiplanar 2D reformations — TRG — are obtained, and cephalometric analysis of teleroentgenograms of the head is performed on them in the Ortho3D program in accordance with the protocol for the study of the facial skeleton. Clinical dental examination is carried out using the "Hamburg test" and the Ilina-Markosyan test, and additionally, computer electrovibrography of the TMJ is performed using the BioJVA device manufactured by Bioresearch, which, using sensors located in the projection of the patient's TMJ, registers vibration signals, arising in the TMJ during the movement of the elements of the joint. The program automatically processes the selected sections of the vibration signal and displays the obtained digital values of the three parameters required to make a diagnosis: “full integral”, “integral at a frequency above 300 Hz”, the ratio of high frequency to low frequency, characterizing the total amount of energy during vibration in the TMJ, to interpret the data and make a preliminary diagnosis, the table-scheme of the analysis of the results attached to the BioJVA device is used, in correlation with the options for the final result of the diagnosis, corresponding to the data of the table-scheme. Ultrasound of the temporomandibular joint is performed with subsequent processing of ultrasound images and construction of the trajectory of movement of the articular heads of the lower jaw during its lowering and raising, surface electromyography of the masticatory muscles is performed using the BioEMG III Bio RESEARCH electromyograph, kinesiography of the lower jaw is performed using the Jaw Tracker 3D Kinesiograph apparatus using various samples in three planes — sagittal, frontal, horizontal and make a diagnosis.
EFFECT: method, due to obtaining sufficient objective data for making a differentiated diagnosis, determining the order and scope of therapeutic measures, makes it possible to make an accurate diagnosis in patients with dentofacial anomalies associated with TMJ disorders and prescribe the optimal treatment.
